You need to complete the 2009 Certification
Examination Program Application
Password
Request Form, and the
Notarized
Application Statement (PDF), and mail them with the
application review fee and other required documents
to the ABMG Administrative Office.
To view and print the forms, you need to have Adobe
Reader.

Upon receipt, ABMG will send you via e-mail your
application number, personal password, and the
secure web address of the online application form
and logbook. Upon receipt of this e-mail, you may
start entering your application details. Note that
the deadline for on-line submission is November 28,
2008.
